Community Health Ambassadors

Neighbors Helping Neighbors: the importance of community ambassadors in promoting good health and addressing barriers to care.

Community Health Ambassadors (CHAs) are an integral part of our outreach efforts. They are city residents who recognize the importance of promoting health and wellness and have decided to do their part to better their community. For New Brunswick, a city that has a predominantly large immigrant, low-income population, there is a lot of distrust of anyone from outside the community. The CHAs have stepped into that space to help engage the community by serving as a cultural bridge between community organizations, Healthcare agencies, and their own respective communities, serving as the bridge to get “in the door.” They help ensure that the message is understood broadly, relevant to the issues they’re dealing with, and next steps are created. In addition, they provide valuable community insights and feedback of the programs to ensure that our projects are well-received in the community.

With that information, our CHAs are much more than community members, they are health promoters that have developed into premiere patient navigators – assisting New Brunswick residents access health and social benefits, including, programs and resources, for needed health care services. As a result, the CHAs help us improve access to healthcare for all New Brunswick residents. It is through their work in the community that the CHAs are able to uniquely help participants overcome system barriers, to truly help create a culture of health in our community.

Our Impact: Engaging with over 500 households and 2500+ residents annually through community outreach events and neighborhood canvassing.

Community Health Hub

The Hub is a central resource connecting New Brunswick residents to healthcare and social services. We offer patient navigation and case management to help eliminate barriers to care. Our team supports individuals with:

  • Choosing a medical provider
  • Scheduling both routine and specialized medical appointments
  • Counseling on health insurance and public benefits, including application assistance
  • Technical support for telehealth and online patient portals
  • Referrals to a wide range of available health and social service programs
Our impact: averaging 20 to 25 people served monthly

Community Wellness Clinics

In partnership with the New Brunswick Public Schools, the New Jersey Black Women Physicians Association  and other community partners, our community health clinics are provided monthly during the flu and cold season providing community members access to the flu vaccine, health screenings and health-related information.

Our impact: averaging over 100 vaccines administered annually

Lead Remediation and Abatement Program (LRAP)

At New Brunswick Tomorrow (NBT), we believe that health and wellness start at home.

A safe, stable home is more than just shelter — it’s the foundation for lifelong well-being. That’s why we focus on the Social Determinants of Health, including housing quality, economic stability, education, and access to care — all key factors that influence a person’s ability to live a healthy life.

One of the most urgent, yet often overlooked, housing-related health issues is lead exposure. Even low levels of lead can cause serious long-term health effects — especially in children, where it can permanently impair brain development. Higher exposure levels can damage the kidneys and nervous system in both children and adults. Homes built before 1978 are particularly at risk, making it essential to test for and eliminate lead hazards.

Through our Lead Remediation and Abatement Program, NBT helps families test their homes for lead and access subsidized funding to safely remove it. We are committed to educating the community, connecting residents with critical resources, and creating safer, healthier homes for all.

Our Impact:

- Canvassed over 500 homes
- Averaging 3 to 5 lead tests conducted weekly
